Tool calls errors like 400

Recently I started experiencing errors like
Error: 400 {"type":"error","error":{"type":"invalid_request_error","message":"invalid params, tool result's tool id(call_function_3w0uqssqt68v_1) not found (2013)"},"request_id":"06076443cb5ff8037625251ceef0e3cc"}
It seems when I start coding session tools calls happens correctly but very quickly after some amount of requests any tool call starting throwing errors like that.
Im using 'pi-mono' as coding agent and have subscriptions to few other providers and I don't see anything like that with them. So I suppose is some bug especially because for weeks before I haven't seen it.
I have one suspicion that this errors depends on amount of tools available in agent, for example if I enable Chrome Dev Tools MCP is much more chances that I will get those errors.
